The University of Minnesota is evaluating the Residential Care Transition Module (RCTM), a 6-session program that takes place over 4 months. This program includes 6 telephone-based or secure web-conference support sessions with a trained transition coach. Sessions can include you or anyone else you would like to include, and each session lasts from 1 to 2 hours.
Family members of relatives with memory loss who are living in a nursing home, assisted living memory care or a similar setting.
Families can often use information and support after a loved one enters a residential setting and this study is designed to evaluate wheterh the RCTM can provide such comprehensive support.
